Thank you for your interest in joining our in-person services! We are looking forward to getting back “to normal” and seeing you again.

We request that guests answer the following questions. Certain responses may require you to avoid attending in-person services at this time. Please know this is simply a temporary health measure for the benefit of our wider community, and not a disinterest in reconnecting with you! We encourage you to join us on livestream if you cannot attend services in-person.

Note: This form does not guarantee you admittance, as we are operating at limited capacity. Forms must be submitted by 6:00 pm Thursday. You will receive a confirmation email from a staff person by Friday morning, confirming your spot. We require you to read and accept our COVID-19 protocols, found here. Thank you for your honest and thorough participation in our survey.

Please be specific, and have each individual in your party fill out a new request form for each service you will join, even if you attend weekly.
This question is being asked on an honour-system. We have many immuno-compromised members who need to feel safe from life-threatening exposure while attending our services in-person. We thank you in advance for being considerate of their health requirements.
